Parents or legal guardians may apply for homebound instruction through the following process:
- Upon either planned or actual school absences due to extended illness or injury, the parent will notify school personnel (such as the school nurse, the school counselor or administrative personnel) of their child's temporary extended illness or injury.
- School personnel will contact the district Home/Hospital (H/H) specialist.
- The District homebound office will consult with school personnel and provide school personnel with the Parent Application Form Packet (Packet).
- The parent will complete the required forms contained within the Packet and submit them to the school.
- The H/H specialist will contact the treating licensed medical provider for a medical recommendation regarding the nature of the illness, its likely duration and other details necessary for the development of an educational plan.NOTE: The Application contains release forms allowing District personnel to discuss the student's condition and needs with the licensed medical provider. Due to this program being based on medical recommendations, the application process cannot proceed until the District is able to secure appropriate information and releases.
- The H/H team will review the application packet, including the licensed medical provider's verification, and eligibility for services.
- If approved by the H/H team, an H/H Instructor will be assigned and a plan will be developed. This plan will typically include up to ten (10) hours of instruction. This instruction time may be individualized according to the student's age, needs and condition.
- In the case of students with disabilities, these temporary services, including the hours of instruction and nature of services, may be individualized in conjunction with the student's Section 504 plan or IEP in order to access the H/H program and any decisions regarding these services will be made in accordance with the requirements of 34 C.F.R. ยง 104.35 based on the individualized assessments of the student's current medical and disability related needs.
- The H/H services will be reviewed periodically, but not less than quarterly, and individualized as determined appropriate by the H/H team, the Section 504 team or IEP team.

- Science
- Social Studies
- The following classes will not typically be supported by H/H:

- Electives
- Science LabsAfter careful consideration of the individual student and the course content, secondary courses in the classes listed in paragraph 2 above may be taught on an H/H basis but only after agreement between the instructor of the homebound student and the classroom teacher. If agreement cannot be reached, the final decision will be made by the principal following a conference between the classroom teacher, instructor and school counselor and may include input from the parent/legal guardian.
- Testing and Grading:

- The teacher of record will provide lecture notes, classroom assignments and tests to the H/H instructor. The H/H instructor will return all completed work and tests to the teacher of record for grading, recording and course credit.
- In cases where the content of the homebound course is not identical to the classroom course, the teacher of record will be responsible for creation of any new tests, assignments or projects and the grading of these tests and assignments. If these tests, assignments, projects or grades are substantially modified, the recorded grade may reflect these modifications in accordance with applicable law.
